This Field RN or LCSW Case Manager (Geriatric Population) role is for a Fortune 500 healthcare company with strong growth potential. This is a full-time contract to hire, travel-based, remote position with face-to-face member visits required.

Field RN or LCSW Case Manager Compensation

• The pay for this position is $38.00-$42.00 per hour PLUS Mileage Reimbursement
• Lodging is reimbursed if needed!
• Benefits available to full-time employees after 90 days of employment.
• A 401(k) with company match available to full-time employees after 1 year of service.

Field RN or LCSW Case Manager Highlights

• Monday–Friday, 8:00 AM–5:00 PM EST
• Field role , with 80% travel required - Travel may be up to 2.5-hour radius from home for in-person member visits.
• Mileage and lodging reimbursement provided.
• Must be based in or near Rocky Mount, NC

Field RN or LCSW Case Manager Responsibilities

• Manage a caseload of members with long-term care needs.
• Conduct assessments with members, caregivers, and providers to determine support systems and care needs.
• Monitor service delivery and follow up via in-person visits and phone contact.
• Authorize and coordinate service referrals.
• Identify deficiencies in care plans and ensure seamless service delivery.
• Collaborate with discharge planners, physicians, and community partners.
• Help members file and resolve complaints and appeals.
• Document visits and maintain records using electronic medical health records.

Field RN or LCSW Case Manager Requirements

• RN or LCSW license required in North Carolina.
• Bachelor’s degree required.
• 2+ years of experience in care management (4–6+ years preferred)
• Experience in physical health, utilization management, long-term care, or home health
• Proficiency in Microsoft Office and EMR software
• Must have a valid driver’s license.
• Attendance is mandatory for the first 90 days.

Field RN or LCSW Case Manager Preferred Qualifications

• Background in home health, discharge planning, or long-term care
• Strong documentation and communication skills
• Ability to work independently while managing a mobile caseload.
