SPECIAL PROCEDURES / UNIT DETAILS: *Completes the initial clinical assessment, reviews clinical history, current diagnoses and presents a clinical summary to the Hospice Medical Director for hospice.*Assesses immediate care needs of the admitted patient and their loved ones, initiates the plan of care and sends assocaited orders to the Hospice Attending provider. Clincal assessment is completed through head to toe eval, interviewing the patient and loved ones, obtaining a medical history through clinical chart review.*Consults with the physician for appropriate orders and assists in obtaining needed supplies, equipment, medications and/or other available services.*Evaluates hospitalized patients admitted for acute care against regulatory criteria and facilitates discharge planning.*Enters clear, concise, and timely clinical documentation related to the patient's initial Hospice admission and assessment.SPECIAL EQUIPMENT: Motor vehicle: ***Must be flexible to travel as needed between different locations, as assigned, in all weather conditions and on roads in varying degrees of repair*** UNIT SYSTEMS: EPIC, SBAR SCHEDULING TYPE: 10 Hour DAYS; 08:00 - 18:30 UNIT ORIENTATION: Approx. 80 hours. Disclosure required (with hospital name/dates employed) at submission; facility approval needed. Current travel Per Diem clinicians aren't eligible for travel contracts they must choose one. Include status and intent in the submission. Local candidates ( 50 miles) : Lower rate applies. RTO : Up to 7 days auto approved (no holidays). Must work 2 of 3 major holidays. No RTO during first week. All RTO must be pre-approved. Cannot request additional RTO after signing. NC or Compact license pending OK for submittal.
