Primary Responsibilities (including, but not limited to):

• · Travel to clients' homes and complete all documentation for nursing assessments and observations
• · Plan evaluations pertaining to clients and caregivers
• · Evaluate clients on a regular basis and/or as necessary to ensure client needs are consistent with the care plan
• · Coordinate with management on implementing and revising operational procedures related to quality client care
• · Utilize professional assessment skills to identify physical, mental, and psycho-social needs of client and be able to develop a care plan to meet these needs in a multi-disciplinary approach
• · Function as a resource and mentor to peers and subordinates to maintain professional nursing standards
• · Act on client’s behalf to assure patient rights
• · Be aware of home safety issues, strive to accomplish all responsibilities in a safe manner, and consistently reinforce good safety attitude and practices displayed by other staff members
• · Document any client complaints concerning a caregiver and review complaints with the management team
• · Coach all staff in proper safety procedures and consistently reinforce procedures

Qualifications:

• · Able to work as a team member
• · Able to maintain confidential information
• · Able to communicate effectively with company staff, clients, client families, and referral sources, community entities
• · Ability to effectively problem solve
• · Able to supervise (instruct, assign work, etc.) caregivers while on duty
• · Excellent verbal and communication skills, adaptable in different situations, and possess excellent client interaction skills
• · Able to multi-task and work independently

Required Education and Experience:

• · Associate Degree in Nursing or Bachelor of Science in Nursing)
• · Current, unencumbered RN license
• · CPR Certification
• · Current TB Test
• · Must maintain CEU requirements as required by the Board of Nursing
• · Valid Driver's License and reliable transportation
