Description

American Traveler is seeking an experienced RN for a Home Health position in the Albemarle, NC area, requiring at least 1 year of home health experience and Homecare Homebase (HCHB) proficiency.

Details

• Home health nursing setting serving patients across Albemarle, Badin, Harrisburg, Mt. Gilead, Norwood, Troy, and surrounding areas
• Patient population includes chronic disease management, wound care, IV therapy, PleurX catheter management, and wound VAC
• Required to complete 6 visit equivalents (VEs) per day, with a minimum of 30 VEs per week
• Nurses are responsible for episode/case management of up to 25–30 patients at a time
• Visits are self-scheduled with patients the day prior
• Average daily driving is 60–80 miles; mileage is reimbursed weekly
• 8-hour day shift, 0800–1700, 40 hours per week
• No weekend rotation required unless the candidate is willing and interested
• No on-call requirement, though on-call shifts may be available for those interested
• EMR: Homecare Homebase (HCHB) — experience required; EPIC Rover used in view-only capacity; Focura used for uploading discharge instructions and wound documentation
• A company cell phone (Samsung Note 9) is provided for documentation and patient/provider communication

Requirements

• Active RN license — NC or compact state license required
• Current BLS certification required
• Valid driver's license and proof of auto insurance required
• Minimum 1 year of home health nursing experience required, with most recent experience within the last year
• Homecare Homebase (HCHB) experience is required — no exceptions
• Proficiency in IV therapy, PleurX catheter care, wound VAC, wound care, patient assessment, disease teaching, and OASIS documentation required
• Ability to manage a caseload of 25–30 patients with full episode management responsibilities
• Last 4 digits of SSN and full date of birth required for consideration
• Minimum 1 professional reference dated within the past year, from a Supervisor, Manager, Director, or Charge RN

Additional Information

• Candidate's permanent address must be located at least 50 miles from the home unit — local candidates are not accepted
• Provides skilled nursing visits including routine care, discharge, resumption of care (ROC), recertification, and reassessment
• Supported by departmental leadership, a scheduler, clinical manager, clinical supervisor, and a collaborative clinical team
• Orientation includes new employee onboarding (NEO) plus 5 days of unit-specific orientation
• Travelers are expected to cover 75% of assigned holidays
• RTO requires manager approval
• Scrubs: navy blue required
• Free parking available on-site
• First-time travelers will be considered if they have solid home health experience
• Former permanent employees of the health system must have been separated for at least 1 year before being eligible
