This position requires proficiency in OASIS documentation and experience with Homecare Homebase software. Deliver compassionate care in a home health setting and help patients achieve their healthcare goals.

Key Responsibilities:

• Conduct thorough assessments and document patient information using OASIS standards
• Develop and execute personalized care plans tailored to patient needs
• Provide nursing care such as wound care, medication administration, and health monitoring
• Collaborate with the healthcare team to ensure comprehensive patient care
• Educate patients and families on managing health conditions and self-care practices

Qualifications:

• Active RN license in North Carolina
• Experience with OASIS documentation and Homecare Homebase required
• 2 years of Home Health experience preferred
• Ability to pass background check, drug screen, and onboarding in compliance with the Joint Commission
• Locals and travelers both accepted
