Join a travel Physical Therapist assignment in Hickory, NC, providing home health services to patients recovering from injuries and managing chronic conditions. This role emphasizes patient-centered care, clinical documentation, and collaboration with interdisciplinary teams to support safe discharges and improved functional outcomes. Local travel between patient homes within assigned coverage areas will be required.
Minimum Qualifications
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by APTA.
- Active North Carolina Physical Therapist license required to start the assignment.
- Current BLS/CPR certification from AHA or ARC required.
- Experience in home health settings preferred, including case management of PT patients and home safety assessments.
- Ability to manage a full caseload, complete timely documentation, and coordinate care with nursing and other disciplines.
- Valid driver's license and reliable transportation for patient visits. Mileage reimbursement provided between visits.
- Minimum education requirement: Bachelor's Degree.
Preferred Qualifications
- Prior travel or contract home health experience preferred.
- Familiarity with LHC-style documentation and coding practices preferred.
- Experience with patient populations in Burke and Caldwell counties preferred.
Assignment Details
- Shift schedule: Monday through Friday, 0800–1600 (start and end times indicated in client details).
- Weekly guaranteed hours: 40 hours.
- On-call and weekend coverage: not required.
- Coverage areas include Burke and Caldwell counties. Local travel between patient homes required.

Impacting Patient Care
