The ISCH staff will provide Individual Support to Massachusetts Waiver Contracted Individuals – regular or intermittent services designed to develop, maintain, and/or maximize the participant’s independent functioning in self-care, physical and emotional growth, socialization, communication, and vocational skills, to achieve objectives of improved health and welfare and to the support the ability of the participant to establish and maintain a residence and live in the community.

Responsibilities:
The ISCH Worker accepts responsibility for his/her own actions and:

  1. To empower individuals to make an informed contribution to any decision about their education, health and care provision to enable them to achieve better outcomes
  2. To assist in the development of information concerning care plans.
  3. To work with and encourage partnership working between individuals and the community.
  4. To work closely with the Interdisciplinary team involved in an individual’s care.
  5. To provide resources to assist the individual in maintaining safety and independent in the community.
  6. To assist with training and supervision of caregivers and PCAs.
  7. To undertake home visits and document activity.
  8. To carry out clerical and administrative duties in accordance with the requirements of the service
  9. Provides necessary information on their own health status and experience.
  10. Accepts only assignments for which qualified.
  11. Attends agency orientation, in-service education programs and staff meetings as required.
  12. Practices safety and competency within the job description.
  13. Communicates with the agency about problems and concerns.
  14. Complies with State/Federal regulatory acts and agency policies.
  15. Communicates with office staff regarding all scheduling of services to clients.
  16. Completes required documentation accurately and on a timely basis.
  17. Respects and maintains client confidentiality at all times.
  18. Reports concerns of client abuse or neglect.
  19. Refrains from discussing personal problems, concerns, or opinions with client, or client representatives.
  20. Establishes and maintains a therapeutic relationship with client.
  21. Maintain access to an email account in order to receive client updates, schedule changes, company updates and highlights.
  22. Maintain a clean, insured vehicle that has had manufacturer’s recommended scheduled services and transports client as appropriate.
  23. To work within the aims and objectives of the goals set in the individual service plan
  24. To be familiar and work within the policies and procedures of the service
  25. To undertake further training appropriate to the role and responsibilities as required
  26. To work flexibly to meet the occasional requirements to work outside normal hours
  27. Maintain confidentiality of sensitive information
  28. Utilize effective time management techniques to ensure tasks are prioritized, and agreed deadlines are met.
  29. Attend all relevant supervision and meetings
  30. To undertake other duties commensurate with this post

Qualifications:

  1. Minimum of a BA in a Human Service-related field preferred.
  2. Have an ability to research and identify other sources of information, advice, guidance and support
  3. Have good IT skills, including managing websites and knowledge of all forms of social/media communications
  4. Have good listening and communication skill
  5. Be able to produce statistics and written reports as required
  6. Be able to travel around the borough independently, preferably by car
  7. Be flexible around working hours and be prepared to work some evenings or weekends as reasonably required by this role
  8. Be impartial and non-judgmental
  9. Have knowledge of the community support system in the Greater Boston area.
  10. Committed to the rights of seniors and disabled people to live dignified lives.
  11. Is flexible, sensitive to the needs of others, reliable, and able to cope with emergencies.
  12. Prior home care experience helpful.
  13. Must be willing to travel throughout Central MA Areas.
