Locum - NP/PA - Geriatric Medicine

Job Title: Geriatric Nurse Practitioner (PACE Clinic)

Job Summary

We are seeking an experienced Nurse Practitioner (NP) for a high-impact role within a PACE Clinic (Program of All-Inclusive Care for the Elderly) . This position focuses on providing comprehensive primary care to frail or elderly participants, helping them maintain their independence within the community. You will work autonomously while collaborating with a Medical Director to manage complex geriatric cases in an interdisciplinary environment.

Schedule & Shift Details:

• Dates: June 15th through TBD (ongoing coverage).
• Shift: Monday – Friday, Day shift (8:00 AM – 5:00 PM).
• Setting: Outpatient PACE Clinic.
• EMR: EPIC.

Key Responsibilities:

• Deliver primary health care and clinical management to a geriatric patient demographic.
• Perform initial and annual comprehensive health risk assessments.
• Develop and implement individualized care plans in collaboration with an interdisciplinary team.
• Coordinate with specialty providers and family members to ensure continuity of care.
• Utilize EPIC for all charting, documentation, and medical record management.

Required Qualifications:

• Certification: Board Certified by ANCC or AANP [Required] .
• Licensure: Active North Carolina State License [Required] (Will consider candidates willing to obtain).
• Experience: * Minimum 1 year of experience specifically working with the frail or elderly population [Required] .
  • Must be an Experienced Provider ; no new graduates will be considered for this role.
• Credentials: Active BLS , CDS Certification , and DEA Registration [Required] .
• Geography: Must be a local candidate. Only mileage (IRS standard rate) is reimbursed; no lodging or airfare provided.

Compensation:
