📝 Job Description:

We’re seeking a detail-oriented and proactive Intake & Triage Specialist to

join our remote team for a limited 3-month engagement. This role is critical in

ensuring accurate and timely processing of patient referrals and service

requests.

Key Responsibilities:

• Review incoming forms for completeness and follow up with stakeholders to gather missing information
• Accurately capture program-specific requirements into CRM systems
• Process forms and requests in alignment with program timelines and guidelines
• Create new patient profiles or link referrals to existing records
• Assess service needs based on form content and apply appropriate business rules
• Collaborate with internal teams to resolve issues efficiently
• Maintain strict confidentiality of patient and proprietary information

Ideal Candidate:

• Strong attention to detail and organizational skills
• Experience with CRM systems and healthcare documentation preferred
• Excellent communication and follow-up abilities
• Ability to work independently in a remote setting
• Commitment to maintaining confidentiality and data integrity
• Comfortable with short-term contract work

Pay and Benefits

The pay range for this position is $16.00 - $16.00/hr.
