Location: Wilmington, NC
Job Type: Full-Time (Onsite)
Schedule: Monday-Friday (Occasional On-Call, Evenings & Weekends as Needed)
Practice Setting: Outpatient Family Practice / Primary Care / Community Health

Job Overview:

We are seeking an experienced Nurse Practitioner or Physician Assistant to provide comprehensive primary care services in a collaborative outpatient setting. This position focuses on preventive care, acute and chronic disease management, patient education, and community-based healthcare. The provider will work closely with physicians and interdisciplinary teams to deliver high-quality, patient-centered care across multiple clinic locations.

Key Responsibilities:

• Perform comprehensive patient evaluations, examinations, and treatment planning

• Diagnose and manage acute and chronic medical conditions

• Order, interpret, and review laboratory, imaging, and diagnostic studies

• Coordinate referrals and collaborate with specialists as needed

• Maintain accurate and timely EMR documentation

• Review laboratory, radiology, and emergency department reports daily

• Collaborate with physicians, nurses, and clinical support staff

• Participate in preventive health screenings and community outreach initiatives

• Assist with development of clinical protocols and standardized care processes

• Participate in occasional on-call coverage and travel between clinic locations as needed

Qualifications:

• Active North Carolina Nurse Practitioner or Physician Assistant license required

• National NP or PA certification required

• 1-3 years of experience in Family Practice, Internal Medicine, Urgent Care, or Community Health

• Strong clinical assessment, diagnostic, and patient management skills

• Excellent communication and documentation abilities

• Ability to work independently and collaboratively within a multidisciplinary team

• Commitment to preventive, patient-centered, and community-based healthcare
