IIH TEAM LEAD:

An Intensive In-Home (IIH) Team Lead is a licensed or provisionally licensed mental health professional (LCSW, LCMHC) who supervises a team of Qualified Professionals (QPs) to provide community-based, clinical services to children and families. They provide individual therapy, lead crisis interventions, develop Person-Centered Plans (PCPs), and provide weekly clinical supervision to team members.

Duties and Responsibilities:

• Providing individual and family therapy for each youth served by the team.
• Designating the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each child.
• Providing and coordinating the assessment and reassessment of the recipient’s clinical needs.
• Providing clinical expertise and guidance to the IIH team members in the team’s interventions with the recipient.
• Providing the clinical supervision of all members of the team for the provision of this service.
• Obtaining input from the supported individuals, providers, and significant others about the service delivery process and seeking information to obtain needed services and supports on behalf of the individual.
• Responsible for maintenance and implementation of a schedule of contacts by staff for each individual being served.
• Responsible for oversight of services received and making certain that the appropriate amount of service is being provided by the service definition.
• Responsible for the development and submission of Treatment Authorization Requests (TARs).
• Responsible for provision on a rotating basis of 24/7/365 First Responder Services for supported individuals and families assigned to the team as required.
• Convening the Child and Family Team for person-centered planning.
• Completing the initial development and ongoing revision of the Person-Centered Plan and ensuring its implementation.
• Monitoring and documenting the status of the recipient’s progress and the effectiveness of the strategies and interventions outlined in the Person-Centered Plan.

Experience: Mental Health services to youth: 1 year (Required).

Education: Master's (Required).

License:

• LCSW.

CST TEAM LEAD:

A Community Support Team (CST) Team Lead is a licensed behavioral health professional who manages a multidisciplinary team. They provide clinical supervision, oversee client Person-Centered Plans (PCPs), and ensure agency services meet strict state and Medicaid compliance standards

Core Responsibilities

• Clinical Oversight & Supervision: Provide weekly clinical supervision to Qualified Professionals (QPs), Certified Peer Support Specialists, and Substance Abuse Professionals.
• Treatment Planning: Develop, monitor, and adjust Person-Centered Plans (PCPs) and Comprehensive Clinical Assessments (CCAs) to ensure medical necessity and positive client outcomes.
• Direct Clinical Care: Deliver direct therapeutic interventions and hands-on support to beneficiaries, often meeting clients in home or community settings.
• Crisis Intervention: Act as the clinical authority to de-escalate crises and coordinate 24/7/365 availability for the team's caseload.
• Documentation & Compliance: Audit and approve clinical documentation, service notes, and billing records to ensure strict alignment with state policies (such as NC DHHS Policy 8A-6) and audit readiness.
• Stakeholder Collaboration: Coordinate care with local Managed Care Organizations (LME/MCOs), hospitals, social services (DSS), and other community partners.

Typical Requirements

• Education & Licensure: Master’s degree in a human services or related field. Requires an active, unrestricted license in North Carolina (e.g., LCSW, LCMHC, LMFT, or LCAS).
• Experience: Typically 1–3 years of experience providing clinical services and case management to adults or children with severe mental illness or substance use disorders.
• Leadership Skills: Strong proficiency in mentoring staff, conflict resolution, and utilizing electronic health record (EHR) systems for treatment data management
