Advance Practice Nurse - (Primary Care - Nurse Practitioner)

Summary

This position is eligible for the Education Debt Reduction Program (EDRP) - a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval - award amount (up to $200 -000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible.

Duties

  • The Primary Care NP will be fully qualified and certified to function in a clinical role and in accordance with the guidelines and policies of the CBOC and the WNCVAHCS
  • This individual is delegated the authority and responsibility in his/her area of expertise to assume major clinical - consultative - educational - research - and administrative duties and assignments related to the examination of claimants filing compensation and pension claims

  • Demonstrates the ability to independently manage a panel of patients with complex - chronic care issues using the best available evidence and national guidelines to achieve optimal population health and effective disease management
  • ii
  • Demonstrates leadership and works collaboratively with assigned Primary Care Team and other interdisciplinary teams to identify - analyze and coordinate internal and external resources to resolve care issues and meet the medical and psychosocial needs of assigned panel of patients

  • Appropriately orders diagnostic and therapeutic tests relative to patient's age-specific needs and health condition
  • iv
  • Appropriately prescribes pharmacologic and non-pharmacologic interventions that support the patient's ability to maintain or regain their health status

  • Monitors and documents the effectiveness of treatment interventions - achievement of institutional quality measures and performance metrics
  • vi
  • Initiates referrals/consults to the appropriate service when indicated vii
  • Sets clinical care goals - short and long term - in collaboration with patient - provider(s) - and family members/caregivers

  • Provides education to patients and their families regarding the complex medical - social - and behavioral conditions with a focus on self-management - prevention and wellness based on the patient's health goals and needs

  • Works closely with the immediate supervisor and nurse manager to complete evaluations in a timely manner with detail to quality

  • Assists with arranging continuity care across the VHA system as needed

  • Works to provide leadership on site to ensure care is completed in required timeframes
