Evaluate, treat, document, and help patients improve mobility, function, independence, safety, and quality of life. Develop and update individualized treatment plans based on patient needs, goals, and progress. Collaborate with rehab leadership, nursing, facility teams, families, and care partners. Support discharge planning, patient education, and functional outcomes. Step into the assignment with professionalism, flexibility, and a patient-centered approach. What We’re Looking For Active or eligible North Carolina Physical Therapist / PT license. Prior skilled nursing, post-acute rehab, LTC, or geriatric experience preferred. Travel experience preferred, but not required for the right clinician. Net Health experience is a plus. Strong documentation, communication, and team collaboration skills.
