Behavioral Health Coordinator BSW
- Identifies and accesses resources to optimize effective and efficient patient behavioral care outcomes in collaboration with other members of the multidisciplinary team.
- Provides care coordination following discharge for patients and their families with various systems, including hospitals, schools, department of social services and other community agencies.
- Assists the patient and families with personal and environmental difficulties which predispose the patient towards illness or interferes with obtaining maximum benefits from medical care.
- Maintains knowledge of and relationships with community health and social agencies to expand the behavioral health services available to patients via collaboration or referral.
- Assists the patient and family to understand medical recommendations and provide services planned to restore the patient to optimum social and health adjustments.
- Assists with paperwork and referral sources for patients being discharged from the hospital.
- Performs other duties as assigned.
- Required: Bachelor's in Social Work or related field
- Required: BLS (CPR)-American Red Cross or AHA Healthcare Provider
