ACT Housing Specialist

Carolina Outreach is seeking a Housing Specialist to provide community-based in-home individual and family interventions to Mental Health and Substance Abuse clients receiving Assertive Community Treatment (ACT) services.

Our comprehensive range of services include school-based therapy outpatient treatment psychiatric evaluations community support and substance use services.

Key Responsibilities:

• Implement therapeutic interventions crisis interventions behavioral interventions psycho-educational activities and other best practice therapies.
• Maintain oversight of services provided within and outside the agency.
• Work with clients families natural supports and collateral agencies in reviewing applying and achieving goals outlined in the person-centered plan.
• Provide parent/guardian/natural support training to improve interaction with the client.
• Assist in all service delivery as formally planned with the clinical team.

Compensation: $21.76 to $24.08

*Final compensation is determined by factors such as education licensure level experience and other qualifications.

Qualifications

• Education: Bachelors degree in Human Services related field required.
