Job Details

???? What You Will Be Doing
Coordinate and manage patient care plans from start of care through discharge
Oversee medication, supply, and equipment needs
Collaborate with interdisciplinary team members (Social Work, Chaplain, PT, OT, etc.)
Present patient updates at team meetings and adjust care plans as needed
Conduct comprehensive patient assessments and plan individualized care
Educate patients, families, and caregivers based on readiness and needs
Promote continuity of care and advocate for patient and family needs

????️ Skills You Need
Active RN license in North Carolina
Graduation from an accredited nursing program
At least 1 year of experience caring for acute or chronically ill patients
Strong communication, assessment, and care coordination skills
