Responsibilities
- Perform clinical assessment to identify patient needs and determine appropriateness for physical therapy services; plan, implement, and evaluate Plan of Care (POC) with ongoing revisions based on evaluation and data.
- Plan and provide treatment for neuromuscular, neurological, and orthopedic disorders aligned with medical referrals and therapy assessments; collaborate with the physician as part of an interdisciplinary team.
- Instruct patients and families on transfer techniques appropriate to patient ability; educate on the safe use of equipment such as walkers, wheelchairs, canes, braces, and prosthetic devices.
- Utilize therapy equipment and order rehabilitative equipment as needed; adapt equipment and appliances to meet home limitations and requirements.
- Provide patient/family education regarding therapeutic procedures, considering age, condition, culture, and learning readiness.
- Document and review clinical records, including POC, progress notes, and related orders; maintain infection control practices and universal precautions in all patient interactions.
- Accept clinical assignments within education and competence; perform other related duties as assigned.

Requirements
- Active state license as a Physical Therapist in North Carolina (Required)
- Proof of current CPR certification (Required)
- Current driver's license with reliable transportation and valid auto insurance (Required)
- 1-2 years of Physical Therapist experience in an acute care or rehabilitation setting (Required)
- Bachelor of Science in Physical Therapy from an American Physical Therapy Association approved program (Required)
- Previous home care experience (Preferred)
