Resident Advocacy Director (Licensed Social Worker) - Navion Senior Solutions

Navion Senior Solutions
Charlotte, NC

Navion is on a mission to elevate health and quality of life for seniors through truly transformative care coordination initiatives – Navion Cares – a standard of care for every resident within the Navion communities. As the Resident Advocacy Director (RAD) at our Assisted Living community in Southeast Charlotte, you will serve as a key support in resident care, assisting with clinical coordination, care management, family communication, and Navion Cares implementation to ensure continuity and quality of services.

You will spearhead enrollment in Medicare care coordination programs, develop and implement personalized care strategies, and ensure cohesive collaboration across interdisciplinary teams and external partners. Additionally, using your unique training and experience, you will provide vitally necessary behavioral health services to community residents.

At Navion, you’re not just joining another senior-care team—you’re part of a movement to empower residents, families, and team members through innovative, mission-driven clinical solutions. Key Responsibilities

You will serve as the central care coordination specialist for residents at our senior living community in Southeast Charlotte, revolutionizing how we deliver clinical support to residents and families.

Program Enrollment & Care Management
Enroll eligible residents in care coordination programs suitable to their specific diagnoses and person-centered care plan goals, then fulfill all program requirements and monthly duties to ensure each care plan runs smoothly and delivers the right level of support.
