Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

Deliver skilled, compassionate nursing care directly in patient homes as a Home Health Registered Nurse (RN) in Fayetteville, NC. This full-time, 13-week contract role offers a dynamic field-based work environment where you will manage a caseload averaging 35 patient visits daily, providing comprehensive care from assessments through education and coordination.

Key Responsibilities:

• Conduct thorough OASIS assessments and develop individualized care plans tailored to each patients needs.
• Administer wound care, medication, IV therapy, and manage chronic diseases with precision and empathy.
• Monitor patient progress, identify changes, and communicate effectively with interdisciplinary teams and physicians.
• Educate patients and families on treatment plans, medications, and preventive care to promote positive health outcomes.
• Maintain accurate documentation using the Home Care Home Base (HCHB) electronic system.
• Collaborate with physical therapists, occupational therapists, medical social workers, and home health aides for coordinated care delivery.
• Participate in case conferences, staff meetings, and continuing education opportunities to stay current on best practices.
• Adhere to regulatory standards and agency policies governing home health care.

Qualifications and Desired Experience:

• Active RN license in good standing within the state.
• Minimum of one year of recent home health experience with field experience preferred.
• Proficiency in Home Care Home Base (HCHB) and experience using OASIS.
• Strong clinical assessment and critical thinking abilities.
• Reliable transportation, valid drivers license, and auto insurance are required.
• Excellent communication, organizational, and time management skills.
• BSN preferred, along with experience managing 2532 patient points weekly.
• Knowledge of Medicare and home health compliance standards is a plus.
