Access To Care Coordinator
SHYS HEALTH formally known as S&H Youth and Adult Services, Inc in Salisbury, NC is looking for one access to care coordinator to join our new management. Our ideal candidate is organized, detail oriented, self-driven, ambitious, and engaged.

- Clinical Supervision and Training for Licensure/Certification. Clinical Supervision for licensure/certification is offered weekly as a benefit of your employment. That time is not paid. Participation in the any of the training and supervision for licensure and certification will require a two-year employment commitment from the commencement date of licensure, certification, and/or supervision and training. Periodic employee contributions come directly out of your paycheck. These benefits are available after you have satisfied your probationary period and you are a permanent employee.
- Paid Time Off (PTO). Paid time off is offered to salaried employees. Employees are able to accrue up to 40 hours of paid time off their first year of service. Please note that all time off request must be submitted at least 14 days/two weeks in advance. These benefits are available after you have satisfied your probationary period and you are a permanent employee.
Responsibilities
Manage ALL intakes :
- Document each step of A2C in the access to care database. Coordinate intake and Assessment for all referrals with intake clinicians. Assign/Complete the initial intake, (diagnostic) assessment and treatment plan/PCP for each new patient, review it and submit it to UM for authorization of services.
- Ensure Patient Access MOAs have entered all info into EHR for all new patients, verified payor source, enrolled uninsured and underinsured patients for state funding, and ensure new admission have completed intake packet, assessment, and PCP/Crisis Plan for each service that has been recommended.
- Ensure the completion of client specific training with assigned intake specialist and assigned counselor/clinician/service upon completion of intake assessment.
- Monitor ADT feeds in collaboration with care management. Ensure all patients hospitalized and admitted to the ED are re-engaged and wrapped with care.
- Ensure the patient begins receiving care/treatment within 14 days of referral.
- Inform referral/referral source via email of the Access to Care timeline. Monitor and ensure submission of ongoing monthly DSS/Probation reports, NC TOPPS.
- Complete 30-day & 90-day follow up to ensure the patient has accessed care.
- Supervise all patient access staff and intake specialist. Ensure access to care, ethical practice, therapeutic documentation, and implementation of evidence-based practice.
Qualifications
- Master Degree in Human Service Field with 2 year post graduate experience with License or Associate License AND
- Is 18 years of age or older with the ability to read, write and understand how to follow instructions and is Licensed with one year of experience with target population AND
- Has no violent criminal record or substantiated findings of abuse or neglect listed on the North Carolina Health Care Personnel Registry
- Experience providing supervision to subordinate staff, counseling, crisis intervention and case management to adults, with severe and persistent mental illness and substance use disorders as well as children/adolescent with severe emotional disorders, substance use and their families.
- Flexibility with scheduling to meet the needs of patients Shift: Mon - Fri (8:30AM – 5:30PM)
