Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

An opportunity is available for an experienced and compassionate Registered Nurse with home health expertise to provide skilled nursing care directly in patient homes. This contract position is based in Lumberton, NC, and involves delivering quality, patient-centered care that supports positive health outcomes.

Key Responsibilities:

• Perform comprehensive OASIS assessments and develop individualized care plans.
• Deliver skilled nursing interventions such as wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify changes in condition, and communicate effectively with healthcare teams and physicians.
• Educate patients and their families on treatment plans, medication management, disease processes, and preventive health.
• Maintain timely and accurate documentation using Home Care Home Base (HCHB) electronic medical record system.
• Collaborate with interdisciplinary professionals including physical therapists, occupational therapists, social workers, and home health aides.
• Participate in case conferences, staff meetings, and professional development activities.
• Adhere strictly to regulatory and agency standards for patient care and documentation.

Qualifications and Experience:

• Active RN license in good standing for the state.
• Minimum of 1 year recent home health nursing experience, with field experience preferred.
• Proficiency with HCHB electronic documentation and OASIS assessment tools.
• Strong clinical assessment, critical thinking, and communication skills.
• Reliable transportation, valid drivers license, and automobile insurance.
• Ability to work independently and manage a flexible schedule.

Preferred Qualifications:

• Bachelor of Science in Nursing (BSN) preferred.
• Experience managing caseloads of 2532 points weekly.
• Familiarity with Medicare and home health compliance standards.

Schedule & Location:

• Full-time contract for 13 weeks.
• Monday through Friday, 8:00 AM to 5:00 PM.
• Field-based with daily travel to patient homes within an assigned territory (mileage reimbursed).
• Caseload typically includes 35 patient visits per day.
