In this role, you will work one-on-one with clients in the comfort of their homes, following individualized care plans designed to support each client's health, safety, and overall well-being. We are looking for someone who leads with empathy, demonstrates patience, and takes pride in delivering attentive, high-quality care while building meaningful relationships with those they serve.

The ideal candidate is detail-oriented, well-organized, and committed to maintaining a clean, safe, and supportive environment for clients. Strong verbal and written communication skills are essential for effectively collaborating with clients, families, and care teams. Previous experience in home health care or a related field is preferred.

Responsibilities

  • Assist patients with activities of daily living (ADLs), including safely helping them move in and out of bed, chairs, and assisting with bathing, grooming, and toileting as needed.
  • Provide personal care support with dignity and respect, ensuring client comfort and privacy at all times.

  • Monitor and document patient care, condition, progress, and any concerns; promptly report observations or changes to the supervisor.
  • Offer companionship by engaging in conversation, reading aloud, and providing emotional support to promote mental and emotional well-being.
  • Provide medication reminders only, in accordance with care plans and company policy (no administering medication).
  • Assist clients with light exercise, mobility routines, or recreational activities to support physical health and independence.
  • Observe and follow all care plans, safety protocols, and infection control procedures to ensure the well-being of both client and caregiver.
  • Maintain effective communication with clients, families, and the care team to ensure continuity and quality of care.
  • Support a safe environment by identifying fall risks, keeping walkways clear, and encouraging the use of assistive devices when needed.
  • Respect client confidentiality and adhere to all HIPAA and company privacy guidelines.

Qualifications

  • High school diploma or GED required
  • Minimum of one (1) year of professional caregiving experience preferred (home health, personal care, or related field)

  • Ability to pass a background check and drug screening

  • Compassionate, patient, and dependable with a genuine desire to help others
  • Ability to follow care plans, document accurately, and report changes in client condition
  • Must be punctual, organized, and able to work independently in a client’s home
  • TB test and/or health screening (if required by state regulations)
