Position Summary

Prolific Community Resources is seeking a dedicated and compassionate Qualified Professional (QP) / Case Manager to provide case management, care coordination, service monitoring, and support for individuals receiving behavioral health and intellectual/developmental disability services. The QP will work closely with members, families, providers, and community partners to ensure services are person-centered and meet identified needs.

Essential Responsibilities

  • Conduct assessments and develop person-centered service plans.
  • Coordinate and monitor behavioral health and community-based services.
  • Maintain regular contact with members, families, providers, and care teams.
  • Complete documentation and case notes in accordance with agency, Medicaid, and regulatory requirements.
  • Participate in treatment team meetings and crisis intervention as needed.
  • Monitor member progress and ensure services are delivered appropriately.
  • Assist members with accessing community resources and supports.
  • Provide after-hours support and on-call coverage as required.

Minimum Qualifications

Required:

  • Bachelor's Degree in Human Services, Psychology, Social Work, Counseling, Sociology, Special Education, or a related field.
  • Minimum 4 years of experience working in the mental health, behavioral health, or developmental disability field.
  • Valid driver's license, reliable transportation, and ability to travel regularly within assigned service areas.
  • Ability to participate in an on-call rotation and respond to urgent member needs.
  • Strong communication, organization, and documentation skills.

Preferred:

  • Experience working with NC Medicaid services.
  • Experience with care coordination, case management, or Innovations Waiver services.
  • Knowledge of person-centered planning and behavioral health service delivery.
