This travel role focuses on home health care, supporting patients across Davidson and Forsyth counties. The position involves working closely with primary care providers to develop and implement comprehensive care plans in a dynamic work environment.

Key Responsibilities of the RN - Case Management

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Reassess cost-efficient, appropriate levels of care continuously
• Put non-medical support in place to ensure treatment compliance
• Coordinate care transitions as needed
• Engage specialty resources to achieve optimal resolution
• Keep detailed records of clinical, functional, and fiscal outcomes
• Identify opportunities for health promotion and illness prevention
• Act as patient advocate by protecting privacy and confidentiality

Requirements of the RN - Case Management

• Valid RN license for NC
• Associates or Bachelor's Degree in Nursing or related field
• Case Management certification
• Experience with Home Health (HCHB experience), Case Management, Oasis (certification preferred)
