RN Case Manager (with $7,500 Completion Bonus)
Registered Nurse – Case Manager Travel Position Location: Kinston, NC Job Type: Travel Contract Duration: 13 weeks Shift: 3×12s Days Pay: $2,228.40/week (plus $2,500 completion bonus per contract, up to $7,500 after three contracts) Job Description We are seeking an experienced Registered Nurse (RN) Case Manager to join a healthcare facility in Kinston, NC on a 13-week travel assignment. The RN Case Manager will coordinate patient care across the continuum, ensuring appropriate utilization of resources and smooth transitions between care settings. The ideal candidate is organized, resourceful, and dedicated to improving patient outcomes while supporting efficient care delivery. Key Responsibilities
- Coordinate and oversee patient care plans in collaboration with physicians and the interdisciplinary healthcare team.
- Assess patient needs and develop individualized care plans to ensure optimal outcomes.
- Facilitate patient discharge planning, arranging post-acute services and resources as needed.
- Monitor patients’ progress and adjust care plans accordingly.
- Serve as a liaison between patients, families, and healthcare providers to support clear communication and care continuity.
- Ensure compliance with healthcare regulations, policies, and standards.
- Document assessments, care plans, interventions, and patient outcomes accurately and promptly.
- Current and valid RN license in North Carolina (or compact eligibility).
- Minimum 2–3 years of recent case management experience in an acute care setting.
- Bachelor’s degree in Nursing (BSN) preferred.
- Current certification in Basic Life Support (BLS).
- Strong assessment, communication, and organizational skills.
- Knowledge of discharge planning, utilization management, and community resources.
- Compassionate and adaptable approach to patient care.
