Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessment of high-risk members
• Collaborate with primary care providers to implement individualized care plans
• Identify and reassess cost-efficient care levels

• Coordinate care transitions and engage specialty resources when needed
• Maintain detailed records of clinical, functional, and fiscal outcomes

Requirements of the RN - Case Management - Travel

• Current, unrestricted state Registered Nurse license or compact license
• Experience in Home Health care, HCHB, and OASIS required
• Preferred Med surg/critical care experience
• Associate's or Bachelor's Degree in Nursing or related field
• Case Management certification
