RN Case Manager Job Details – Greensboro, NC

• Shift: Evening Shift, Monday – Friday, 3:00 PM – 11:30 PM

• Contract Duration: 13 weeks (Start: 10/7/2025 – End: 1/6/2026)
• Estimated Weekly Salary Range: $2,250 - $2,310 per week

Registered Nurse Case Manager Qualifications

• Active Registered Nurse (RN) license (North Carolina or compact state, must be in hand at submission)
• Minimum 2 years of recent acute care hospital RN experience (no first-time travelers)
• BLS certification (current and valid)
• Previous charge nurse experience required
• Experience in ICU, Med-Surg (MS), PICU, SDU, PCU, IMC, or Observation units
• Proficiency with EPIC electronic health record system
• Case management and utilization review experience, including discharge planning, needs assessment, DME ordering, and plan of care development
• Knowledge of CMS (Centers for Medicare and Medicaid Services) regulations
• Must pass EKG exam during compliance (2 attempts allowed)
• COVID-19 and Flu vaccinations required (medical/religious exemptions considered)
• Willingness to float to other hospital campuses as needed
• Education verification and background check required
• No local candidates accepted (must reside 50+ miles from facility)

RN Case Manager Responsibilities

• Coordinate and manage patient care across the continuum to ensure optimal clinical outcomes and efficient resource utilization
• Conduct comprehensive needs assessments and develop individualized plans of care for patients
• Facilitate discharge planning, arrange for DME (durable medical equipment), and coordinate post-acute services
• Perform utilization review and ensure compliance with CMS and hospital policies
• Collaborate with interdisciplinary healthcare teams to support patient transitions and care coordination
• Accurately document all case management activities in the EPIC system
• Participate in regulatory compliance and quality improvement initiatives within the hospital
• Provide leadership and support as a charge nurse when required
