Bachelor of Science in Nursing (BSN) degree, active registered nurse (RN) license and minimum 1+ years RN experience required. Applicants who do not meet these qualifications will not be considered.

A skilled and compassionate Home Health Registered Nurse (RN) is needed to provide high-quality nursing care in patients homes. This role demands clinical expertise, strong communication, and the ability to work independently within a flexible, field-based schedule. You will perform comprehensive patient assessments, develop individualized care plans, and collaborate closely with interdisciplinary healthcare teams to ensure optimal patient outcomes.

Key Responsibilities:

• Conduct thorough OASIS assessments and create patient-specific care plans.
• Deliver skilled nursing care including wound care, medication administration, IV therapy, and chronic disease management.
• Monitor patient progress, identify condition changes, and communicate promptly with healthcare providers.
• Provide education to patients and families regarding treatment plans, medications, and preventive care.
• Maintain accurate documentation using Home Care Home Base (HCHB) software.
• Collaborate with therapists, social workers, and home health aides for coordinated care delivery.
• Participate in case conferences, staff meetings, and continuing education.
• Adhere to all regulatory and agency standards for documentation and care quality.

Qualifications and Desired Experience:

• Active RN license in the state of practice (good standing).
• Minimum 1 year recent home health experience, preferably field-based.
• Proficiency with Home Care Home Base (HCHB) and OASIS assessment tools.
• Strong clinical assessment and critical thinking skills.
• Reliable transportation, valid driver's license, and auto insurance.
• Effective communication, time management, and organizational abilities.
• BSN preferred.
• Experience managing a caseload of 2532 points per week is advantageous.
• Knowledge of Medicare and home health compliance standards.

Schedule:

• Full-time contract, 13 weeks.
• Monday through Friday, 8:00 AM 5:00 PM.
• Caseload typically involves 35 patient visits per day within an assigned territory.
