Intake Specialist
S&H Youth and Adult Services in Salisbury, NC is looking for one Intake Specialist to join our strong team. Our ideal candidate is attentive, ambitious, and reliable.
The Intake Specialist must be a master level Licensed or Associate Licensed Qualified Professional with at least one year of experience with the target population. An Intake Specialist is a clinician who is responsible for providing clinical intakes, initial assessments, re-assessments, and treatment planning. Intake clinicians are also responsible for initial crisis plan development and completing progress notes daily for work completed.
Responsibilities
- Conducts comprehensive clinical assessments to identify presenting problems, biopsychosocial history, substance use history, the status of social determinants of health, service needs, and life goals for adults, adolescents, and children.
- Coordinates with the collateral (natural supports/referral agent) and gathers information from community resources which may include schools, medical providers, hospitals, courts.
- Develop, coordinate, and implement new patient treatment plans including coordination of first-time appointments for therapy, medication clinic, peer support, and other mental health and substance use treatment services.
- Complete daily progress notes within 24 hours of service and submit on Mondays by 12pm to HR.
- Maintain current certification/license in all required training as outlined by agency policy and/or state and federal regulations according to discipline and service.
Qualifications
- Licensed or Associate Licensed Clinician: LCHMC, LCMHCA, LCSW, LCSWA, LMFT, LMFTA, LCAS, LCASA.
