Nurse Case Manager RN
Position Specific Information:
This Nurse Case Manager position will work closely with Services For Adults (SFA) social workers, including Adult Protective Services, Guardianship, Community Social Workers, the Economic Services Division, and SOAR by conducting in-home visits and medical records reviews for clients who have reports of neglect and/or abuse.
Minimum Qualifications:
- Experience: Minimum of two years of nursing case management experience.
- Education: Bachelor’s degree or higher in Nursing from an accredited college. BSN or MSN.
- Licenses and Certifications:
- Current license to practice as a Registered Nurse (RN) in NC issued by the NC Board of Nursing or another Nurse Licensure Compact State.
- Certification in Basic Life Support for Healthcare Providers (CPR & AED) by the American Heart Association that is valid for at least 90 days after hire. Maintenance of this certification is required and as funds are available may be supported as part of employment.
- Requires a valid North Carolina or South Carolina Driver's License.
- Requires County Driving Privileges.
- Requires an Annual Motor Vehicle Record (MVR) check required.
Preferred Qualifications:
- Previous nursing experience with adult populations.
- Previous experience with mental health populations.
- Previous experience with Medical Record Reviews.
- Previous experience with In-Home and community-based provided services.
