Your Role: UR / Patient Care Coordinator (RN)

This is a key coordination role overseeing both Utilization Review and our Transitional Care (Swing Bed) Program.

What Youll Do:

Coordinate and manage the Swing Bed (Transitional Care) program
Review admissions for medical necessity and regulatory compliance
Manage referrals (internal & external) and build provider relationships
Collaborate with physicians, case management, and care teams
Support discharge planning for safe, timely transitions
Conduct utilization reviews to optimize length of stay
Handle pre-authorizations and follow up on denials
Ensure compliance with all regulatory and accreditation standards

Schedule

• Full-time (40 hours/week)
• 1st shift | MondayFriday
• Occasional evenings/weekends as needed
• No travel required

What You Bring

• Active RN license (North Carolina)
• BLS Certification
• Utilization Review experience (required)
• Strong communication and coordination skills
• Ability to manage multiple priorities and systems

Preferred Experience

• Swing Bed / Transitional Care coordination
• Case management or discharge planning
• Hospital-based experience
