Registered Nurse Qualifications

• Minimum 2 years of recent experience as a Home Health Registered Nurse (RN)
• Proficient with OASIS and HCHB (Homecare Homebase) documentation systems
• Active RN license in the state of North Carolina
• Current CPR certification
• Valid driver’s license, vehicle insurance, and reliable transportation for travel nursing assignments

Home Health RN Responsibilities

• Provide compassionate, high-quality nursing care to patients in their homes throughout the Durham, NC area
• Perform comprehensive patient assessments and develop individualized care plans
• Accurately document patient care using OASIS and HCHB systems
• Collaborate with interdisciplinary healthcare teams to achieve optimal patient outcomes
• Educate patients and families on care procedures, health management, and wellness strategies
• Travel to patient residences within Durham and surrounding North Carolina communities
