Summary of Position

The Psychiatric Mental Health Nurse Practitioner (PMH-NP), under the supervision of an assigned psychiatrist, is responsible for assisting in the delivery of psychiatric health care and patient care management to adult and/or pediatric patients. Under a psychiatrist’s supervision, the PMH-NP is responsible for the delivery of a broad range of psychiatric medication management services, including taking medical and biopsychosocial histories, conducting psychiatric assessments, ordering labs as clinically indicated to support medication decision-making, monitoring patients, prescribing medications, and counseling and educating patients on their treatment plan. The PMH-NP will work as part of a collaborative and integrated system, working closely with the Chief Medical Officer and Chief Behavioral Health Officer.

Minimum Qualifications

Current unrestricted certification and/or licensure

• Previous psychiatric medication management and FQHC experience preferred.
• CPR, and all applicable certifications required (must be obtained within 30 days of employment).
• Knowledgeable in population health management in the underserved and/or vulnerable populations
• Understands ethics of confidentiality.
• Previous computer experience or training required.
• Successful candidates must submit to TB testing and any vaccinations recommended by OSHA Guidelines.
• Sets an example as a psychiatric professional by observing work start and stop times, giving adequate notice for absences, and following the clinic’s operational and personnel policies and procedures.
• Able to communicate and relate well with patients, providers, and clinical staff.
• Maintains current and unrestricted licensure and certification, including meeting and/or exceeding CME requirements.
• Observes standards of medical and psychiatric care.
• Efficient, organized, and accurate.
• Ability to sit and stand for long periods of time
• Frequent walking for short distances
• Repetitive motion work with keyboards and some limited physical activity

Skills

• Excellent written and oral communication skills
• Proficiency in Microsoft Office Outlook, Word, Excel, PowerPoint, Access, and use of e-mail communication
• Problem analysis and problem solution at both a strategic and functional level
• Ability to communicate clearly and succinctly

Education:

• Graduation from an accredited program
• Current State of North Carolina NP licensure/certification required, in good standing with the Nursing Board
• Current DEA certificate required
• Board Certification if applicable.
  

K ey Responsibilities (10-Core)

Engages in a patient-centered approach to providing care for patients and their families

1. As a member of an interdisciplinary health care team, provides consultative services within the scope of care definitions and directs the provision of support services to organization in screening, diagnosing, managing, and treating mental illness .
2. As a member of an interdisciplinary health care team, works in collaboration with behavioral health providers and behavioral health support staff
3. Provides psychiatric assessment, evaluation, and treatment using evidence-based procedures as part of the clinical care team. Responds to psychiatric emergencies and answers psychiatric medication management questions.
4. Obtains and documents, or causes to be obtained and documented; appropriate chief complaint, past medical, past psychiatric, family and social history, review of systems, examination, medications and allergies, assessment and plan for each patient. As needed and appropriate, implement behavioral health screening tools, assess for safety, and provide patient education.
5. Provides psychiatric medical orders for prescription refills, administration of medications, ancillary testing, and/or referrals as appropriate to meet patient need
6. Reviews ancillary test results in a timely manner and coordinates notification and follow-up to patients.
7. Supervises patient care by external agencies such as specialty providers, home health and psychiatric care, enhanced psychiatric services, nursing home, or hospice.
8. Available to patients personally or through clinical staff to answer questions and relay information regarding their care.
9. Completion of all appropriate paperwork in a timely manner, including documentation to comply with insurance and reimbursement guidelines.
10. Educates patients and/or families as to the nature of disease, provides instruction on proper care, self-management and treatment, and works with patient to develop an individualized care plan.
11. Actively participates in the QI process by reviewing peer charts when appropriate and gives feedback to peers to support a culture of peer education and continual learning
12. Perform other duties as assigned by his/her supervising physiatrist, the Chief Medical Officer, the Chief Behavioral Health Officer, Practice Manager and/or members of the Kintegra Health executive leadership team.
