As an Admissions Nurse (RN) , you are the first clinical impression families and residents experience. Your skill, compassion, and professionalism set the tone for a smooth transition into our community.

Job Summary
We are seeking a skilled and compassionate Admissions Nurse (RN) to join our team. This role plays a key part in welcoming new residents, coordinating admissions, and ensuring a seamless transition into our facility.
This position requires strong clinical assessment skills, attention to detail, and a resident-centered approach.
✅ Responsibilities
- Conduct initial clinical assessments of new residents
- Develop individualized care plans based on assessment findings
- Coordinate with families, physicians, hospitals, and other healthcare providers
- Ensure all required documentation is completed accurately and timely
- Educate residents and families on services, policies, and care processes
- Collaborate with the interdisciplinary team for continuity of care
- Provide emotional support during the transition process
- Participate in quality improvement initiatives related to admissions
Qualifications
- Current Registered Nurse (RN) license in good standing
- 1–2 years of nursing experience preferred (long-term care or geriatrics a plus)
- Strong assessment and documentation skills
- Knowledge of state and federal nursing home regulations
- Excellent communication and interpersonal skills
- Ability to work independently and collaboratively
- Compassionate, patient-focused approach
