Travel Licensed Practical Nurse LPN Job
CWHH-LPN-LVN - Licensed Practical Nurse (LPN/LVN) - Greensboro, North Carolina, United States. Start date: 03/16/2026. Duration: 13 weeks. Weekly pay: 1,418 to 1,492 USD per week. - Working in a home health/hospice care setting while traveling Responsibilities
- Under the direction of the RN, administer skilled nursing care to patients in their place of residence, as prescribed and in accordance with the plan of care; coordinate with the interdisciplinary team and patient/family.
- Participate in planning, coordinating, and implementing care with the RN to deliver quality, cost-effective patient care.
- Communicate effectively with patients, families, co-workers, and other health professionals; document outcomes of interventions in clinical notes and evaluations.
- Assist patients and families in learning appropriate self-care techniques; assess responses to care and adjust care plans as needed.
- Maintain infection control measures to protect staff and patients (OSHA-compliant).
- Assist the RN and/or physician with specialized duties related to end-of-life care for Hospice patients; assist with admitting, discharging, and transferring patients as needed.
- Potential on-call duties.
- Maintain professional boundaries and recognize unsafe conditions, reporting and addressing issues promptly; participate in annual in-service and performance improvement activities. Requirements
- Current LPN/LVN license in the practicing state (North Carolina).
- Current CPR Certification.
- Valid Driver’s License with reliable transportation; ability to use electronic medical charting and time-keeping software (Homecare Homebase).
- Proof of Hepatitis consent/declination and TB vaccination record.
- Minimum two years’ experience as an LPN/LVN in a clinical setting, preferably in home health or hospice.
- Education: Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
- Must be able to read, write, and speak fluent English; good attendance required. Benefits and Support
