Care Manager
Hope Services, LLC is seeking a Care Manager in Wake County within our Tailored Care Management Department.
Applicant must be a qualified professional (not licensed or provisionally licensed). The primary responsibilities of the Care Manager are:- Assisting patients in navigating and accessing health care.
- Promote whole person care.
- Coordinate and assist in integrating care with patient's entire health care team.
- Assess patients SDOH needs and make appropriate referrals.
- Closing gaps in monitoring care to individuals and achieving key goals of the tailored plan related to behavioral health, mental health, and physical health.
- Must be comfortable with direct patient contact, including in-home and community appointments.
- Experience with NC Tailored Care Management is required.
The Care Manager shall report to the Care Manager Supervisor.
