Job responsibilities:
- Document evaluations, treatments, and outcomes in the medical record.
- Evaluate functional status and develop a treatment plan aligned with clinician goals and patient needs.
- Work 40 hours per week with 8-hour shifts.
- Perform reassessments and update the plan of care based on patient response.
- Provide therapeutic interventions and track patient progress toward measurable goals.
