About the Role:
The Home Care Supervisor plays a critical role in ensuring the delivery of high-quality home health services to patients in their residences. This position is responsible for overseeing the care team, coordinating patient care plans, and ensuring compliance with healthcare regulations and standards. The supervisor will work closely with healthcare professionals, patients, and their families to provide education and support, fostering a collaborative environment that prioritizes patient well-being. Additionally, the Home Care Supervisor will monitor patient progress and adjust care plans as necessary to meet evolving health needs. Ultimately, this role aims to enhance the quality of life for patients while ensuring that care is delivered efficiently and effectively.
Minimum Qualifications:
- Registered Nurse (RN) license with a minimum of 3 years of experience in home health or a related field.
- Proven experience in case management and patient care coordination.
Preferred Qualifications:
- Bachelor's degree in Nursing or Healthcare Management.
- Certification in Home Health Care Management or a related specialty.
Responsibilities:
- Supervise and manage a team of home health aides and nurses, providing guidance and support in daily operations.
- Develop, implement, and evaluate individualized care plans for patients, ensuring they meet their specific health needs.
- Conduct regular assessments of patient conditions and progress, making necessary adjustments to care plans.
- Ensure compliance with all healthcare regulations, policies, and procedures, maintaining high standards of care.
- Provide education and training to staff and patients regarding health management, safety protocols, and best practices in home care.
