About the Role:

As a Coumadin Clinic Coordinator, RN, your major end result will be to provide comprehensive care and coordination for patients receiving Coumadin therapy. You will play a crucial role in ensuring the safe and effective management of anticoagulation therapy, helping patients achieve optimal outcomes and minimize complications. Your expertise and attention to detail will be essential in monitoring patients' response to therapy, adjusting dosages as needed, and educating patients on the importance of adherence to treatment plans. By effectively coordinating care with other healthcare professionals, you will contribute to the overall success of the Coumadin Clinic and enhance patient satisfaction.

Minimum Qualifications:

• Valid Registered Nurse (RN) license in North Carolina
• Bachelor's degree in Nursing (BSN) or equivalent.
• Minimum of 2 years of clinical experience in an acute care setting.
• Experience in anticoagulation therapy management, including knowledge of Coumadin dosing and monitoring.
• Strong communication and interpersonal skills, with the ability to effectively educate and engage patients.

Preferred Qualifications:

• Certification in Anticoagulation Management (ACM) or equivalent.
• Experience working in a Coumadin Clinic or similar anticoagulation management setting.
• Knowledge of electronic medical record (EMR) systems and proficiency in documentation.
• Ability to multitask and prioritize responsibilities in a fast-paced environment.
• Demonstrated commitment to continuous learning and professional development.

Responsibilities:

• Coordinate and manage the daily operations of the Coumadin Clinic, including scheduling appointments, triaging patient calls, and ensuring timely follow-up.
• Conduct thorough assessments of patients' medical history, current medications, and laboratory results to determine appropriate Coumadin dosages.
• Monitor patients' response to therapy through regular INR testing, evaluate results, and make necessary adjustments to dosages.
• Provide patient education on Coumadin therapy, including medication administration, dietary considerations, and signs of potential complications.
• Collaborate with healthcare providers, including physicians, pharmacists, and laboratory staff, to ensure seamless coordination of care and optimize patient outcomes.

Skills:

In this role, your required skills in clinical assessment, medication management, and patient education will be utilized on a daily basis. You will need to accurately assess patients' medical history, laboratory results, and medication profiles to determine appropriate Coumadin dosages. Strong communication skills are essential for effectively educating patients on the importance of adherence to treatment plans and providing clear instructions on medication administration and dietary considerations. Additionally, your ability to collaborate and coordinate care with other healthcare professionals will be crucial in ensuring seamless patient management.
