Community Integration Coordinator

Department : IMPACT Team – 1915(i) Medicaid Services

Reports To : QP Supervisor

Location : Office-Based, Field-Based

Employment Type : Part-Time | Hourly ($15–$17/hr)

POSITION SUMMARY:

  • The Community Integration Coordinator is the operational lead of the IMPACT Team, responsible for delivering 1915(i) Medicaid services directly to clients while managing documentation, compliance, coordination of care, and connection to peer support, housing, and employment.
  • This role is ideal for someone with strong organizational skills, lived or professional experience in social services, and the ability to manage both people and paperwork.

KEY RESPONSIBILITIES:

Direct Client Services:

  • Deliver Individual Support and Community Living & Support through one-on-one sessions in the home or community.
  • Provide Supported Employment by assisting clients with job readiness, applications, resumes, and workplace support.
  • Assist with Community Transition by helping clients prepare for housing, secure units, gather supplies, and move in.

Documentation & Compliance:

  • Draft the Person-Centered Plan (PCP) for each client, ensuring accuracy, goals, and service alignment across all 1915(i) codes.
  • Maintain daily progress notes , time-in/out, service type, and modality.
  • Upload and organize documentation in EHR.
  • Track units per client to avoid billing errors or exceeding caps.
  • Keep updated logs of employment search, housing leads, and service delivery.

Team & Workflow Management:

  • Manage your weekly schedule to balance client visits, admin time, and outreach.
  • Coordinate services with Peer Support and OPT providers to avoid overlap.
  • Participate in weekly team huddles with the QP Supervisor.
  • Maintain and update the IMPACT Team Tracker Spreadsheet .

Client Advocacy & Outreach:

  • Support client navigation of systems: housing authorities, employers, landlords, job boards.
  • Escort or arrange transportation for job interviews, housing inspections, or community services.
  • Attend community partner meetings or outreach events to expand referral base.


Administrative Time & Travel Support (Gas):

This position includes both billable and non-billable responsibilities. Direct service delivery with clients is billable.

However, certain tasks such as weekly team huddles, planning meetings, compliance check-ins, documentation prep, and coordination require administrative, non-billable time . These hours will be paid at the North Carolina minimum wage of $7.25/hour , with a maximum of 6 administrative hours per week .

To offset local travel and support community-based work, this role also includes a $25/week gas stipend , issued monthly. This stipend is intended to help cover fuel costs incurred while supporting clients in the field.

QUALIFICATIONS:
Required:

  • High School Diploma or GED (Associate's Degree preferred).
  • 1–2 years of experience in human services, case management, job coaching, peer support, or community programs.
  • Strong written communication and time management skills.
  • Comfortable working independently in the field.
  • Basic Microsoft Excel and Google Workspace proficiency.

Preferred:

  • Knowledge of 1915(i) or Medicaid-funded services.
  • Experience supporting individuals with mental health, substance use, or housing instability.
  • CPR/First Aid, Crisis Intervention, or Mental Health First Aid certifications.

KEY PERFORMANCE INDICATORS (KPIs):

  • Minimum 20 billable hours per week .
  • Timely submission of notes and PCPs (within 24–48 hours).
  • Accurate tracking of client progress and unit limits.
  • Compliance with Medicaid service definitions and documentation standards.
  • Successful Community Transition placements and employment milestones.
