What You’ll Need
- NYS RN license required
- Associate’s Degree in Nursing (experienced candidates) or BSN (new graduates)
- 1 year of RN experience in managed care, home care, acute care, or nursing home preferred
- Certified Case Manager (CCM) preferred
- Bilingual proficiency required
What You’ll Do
- Provide case management for members, addressing medical, social, psychological, physical, and spiritual needs
- Develop, implement, and monitor care plans in collaboration with PCPs, caregivers, and the care team
- Ensure high-quality, personalized care for all members
- Support the goals and values of the managed care health plans
