Pharmacy Manager - Home Infusion (Wilmington, NC)

Key Responsibilities
- Conduct comprehensive physical assessments and review medical histories to evaluate patient conditions.
- Develop and execute personalized treatment plans that align with each patient's unique rehabilitation goals.
- Administer therapeutic exercises and training to improve patients' functional skills and independence.
- Educate patients and their families on injury prevention, wellness strategies, and recovery steps.
- Maintain accurate documentation to track patient progress and rehabilitation outcomes.
- Collaborate with a multidisciplinary team to ensure holistic and effective patient care.
Qualifications
- Graduation from an accredited Physical Therapy Program.
- Experience in Home Health care is required.
- A minimum of nine months of professional experience as a Physical Therapist.
- Outstanding interpersonal skills with a compassionate approach to patient interactions.
