Care Navigator

Position Summary
Anuvia is seeking a dedicated Care Navigator to join our Clinical Outpatient team. This role is central to ensuring our clients receive safe, effective, and client-centered care. The Care Navigator monitors client needs, coordinates treatment plans, connects clients with community resources, facilitates care transitions, and advocates for client well-being. This position also supports admissions, service authorizations, and documentation requirements.
Key Responsibilities
Coordinate and deliver care that is timely, effective, equitable, and client-centered.
Conduct client admissions, obtain service authorizations, and develop person-centered treatment plans.
Review assessments and referrals to ensure appropriate service connection.
Monitor treatment progress, signs of substance use withdrawal, and adjust care as needed.
Facilitate discharge and transition planning.
Provide education on substance use, health, and community resources.
Assist clients with applications and service-related documentation.
Build and maintain strong relationships with referral agencies and community partners.
Maintain accurate, timely documentation in accordance with state, agency, and federal guidelines.
Administer medications per provider orders (training provided) and take vital signs as directed.
Adhere to all ethical standards, codes of conduct, and agency protocols.
Participate in staff meetings and multidisciplinary team discussions.
Qualifications
Bachelor’s degree preferred, or equivalent experience/training.
Must meet criteria for Qualified Professional and maintain certification with the North Carolina Addictions Specialist Professional Practice Board and/or North Carolina Department of Health and Human Services .
Valid NC or SC Driver’s License required.
Strong interpersonal, problem-solving, and communication skills.
Ability to document in Electronic Health Records and use Microsoft Office applications.

Work Environment & Schedule
- 8am-4:30pm M-F
Based in our Clinical Outpatient department with community-based work as needed.
Standard schedule with potential variation based on client needs.
