What You'll Do

As our MDS Coordinator , you'll play a key role in ensuring accurate assessments, regulatory compliance, and quality resident care.

Responsibilities include:

• Coordinate and oversee the MDS, CAAs, and Care Planning process.

• Collaborate with the interdisciplinary team to complete resident assessments.

• Manage MDS scheduling for Medicare, Medicaid, and other payer sources.

• Lead or participate in PPS, Medicare, and reimbursement meetings.

• Ensure assessments meet all federal and state regulatory requirements.

• Review clinical documentation and perform chart audits.

• Conduct resident assessments through observation, documentation review, and resident/family interviews.

• Support quality improvement initiatives and accurate reimbursement.

• Partner with nursing leadership to maintain compliance and exceptional resident outcomes.

Qualifications

We're looking for someone who has:

• Current North Carolina RN or LPN license .

• At least 3 years of clinical nursing experience (long-term care or skilled nursing preferred).

• MDS experience strongly preferred.

• Current CPR certification.

• Strong organizational and communication skills.

• Knowledge of CMS regulations, Medicare reimbursement, and long-term care standards.

• A commitment to resident-centered, quality-driven care.
