GENERAL DESCRIPTION
This position is a fast paced, comprehensive position that requires the ability to multitask, enlist community resources, change approach at a moments notice and work as a team member on behalf of the individuals we serve.

ENVIRONMENT
This is a professional position, based in the Clemmons Office where all Guardian Representatives are expected to participate in a daily in person staff meeting at the start of each day. This position will share office which can be distracting as well as beneficial. While things can get stressful, laughter and peer support offer outlets and resources.

DISTINGUISHING FEATURES
This is advanced level human service work involving the provision of direct and indirect casework for adults requiring temporary or ongoing assistance with decision making for their day to day life. The Guardian Representative must be able to analyze a situation and determine an appropriate course of action in a short time span. Occasionally we assist MH/IDD/SU providers in emergency situations, it is the exception and not the rule.

Each Guardian Representative is assigned a specific group of people to work with, but it is not uncommon for your schedule can change at any time to help a co-worker or make a last minute court appearance. All employees are expected to dress, interact and communicate professionally. This position is office based and you will be expected in the office Monday- Friday from 8:30am-5pm unless the schedule dictates an alternative location.
This is a heavy paperwork position that requires attention to detail.

EXAMPLES OF DUTIES:
  • Serve as an advocate and substitute decision maker for those individuals appointed by the judicial system and assigned by the agency.
  • Coordinate services with county Departments of Social Services, clerk of courts, their family, MH/IDD/SU providers, and/or any other governmental or private agency involved with the individual.
  • Clearly communicate with service providers outside of Empowering Lives Guardianship Services LLC.
  • Complete comprehensive assessments of disabled adults' situations to determine services needed.
  • Facilitate care management to arrange/coordinate an array of services such as medical, emergency, psychological, financial (representative payee), family consultation and court systems that will help the individual meet daily needs, remain safe, and optimize their functional abilities despite high risk behavior.
  • Coordinate crisis stabilization for behavioral disruptions and emergency situations utilizing available resources.
  • Collaborate with Mental Health providers and Managed Care Organizations (MCO) to ensure mental health, developmental disability and substance use services are provided and meet the need(s) of the individual.
  • Understanding and knowledge of community resources for all ages over 18, navigating the criminal court system, and knowledge of accessing medical and mental health services is a must.
  • Adhere to state/contract mandated procedures/guidelines.
  • Skills and comfort with heavy use of the computer, internet and Smart Phone is required. Specific examples include Microsoft Office, IDrive, scanning, online faxing and online documentation.
  • Proficient documentation skills and use of language.
  • Familiarity with DSM 5 and the correlation between diagnoses, symptoms, medications, and needs.
  • Attend and participate in daily staffing meetings and report progress to facilitator.
  • Attend and participate in MH/IDD/SU meetings regularly in person.
  • Visit with the individual at least every 90 days with two of those being at their residence or home.
  • Complete daily documentation (including daily activity logs), complete assessments, write treatment plans, write and file court reports and any other required documentation as assigned.
TYPICAL QUALIFICATIONS:
Minimum Qualifications
  • Four year degree in a human service field from an accredited college/university and two years directly related experience minimum post bachelorette.
  • Must have possession of an appropriate driver's license issued by the State of North Carolina.
  • A motor vehicle record (MVR), background check, and drug screening will be conducted on candidates considered for positions.
  • Ability to work from the office M-F 8:30AM - 5:00PM most days.

Physical Demands and Work Conditions
This is position requires a customer service oriented ability to interact with others. The ability to evaluate a room or meeting before engaging is necessary. This work requires contact with the individuals assigned, family members and other professional disciplines and service agencies. Employees may experience emotional stress due to the nature of interactions with clients or service providers and behavioral problems such as; aggression of clients and/or family members that cause bodily injury for employees.

Employees work from the office in Clemmons, but spend some time traveling to into the community such as public service buildings, residential facilities, hospitals, institutions, home environments, and homeless shelters.

  • Must possess the visual acuity to prepare and analyze data, operate a computer, and do extensive reading.
  • Must be able to physically perform the basic life operational functions of climbing, balancing, stooping, kneeling, crouching, crawling, reaching, standing, walking, pushing, pulling, lifting, fingering, grasping, feeling, talking, hearing, and repetitive motion.
  • Must be able to perform medium work exerting up to 50 pounds of force occasionally and/or up to 10 pounds of force constantly to move objects.
