Key Responsibilities
- Conduct and coordinate comprehensive assessments of new and existing residents using the MDS tool
- Develop, implement, and review individualized care plans in collaboration with an interdisciplinary team
- Ensure compliance with federal and state regulations for MDS assessments and care planning
- Monitor and transmit MDS assessments to the relevant authorities
- Train and guide nursing staff on MDS procedures and updates
- Audit clinical records to ensure accuracy, completeness, and regulatory compliance
- Collaborate with healthcare providers to optimize resident care outcomes
Minimum Qualifications
- Active RN license required
- MDS Certification preferred
- Proven experience in clinical assessment and MDS coordination
- Strong knowledge of federal and state regulations related to MDS and care planning
- Exceptional organizational skills and attention to detail
- Excellent communication and interpersonal skills to interact with staff and residents
