Travel Registered Nurse Home Health Job
Registered Nurse – Home Health in Raleigh, North Carolina. Travel assignment begins 03/12/2026 for 13 weeks with 40 guaranteed hours per week on an 8-hour shift. Weekly pay ranges from $2,049 to $2,187. The role delivers skilled nursing care to clients in their place of residence, coordinating with an interdisciplinary team and referring agencies to ensure quality and continuity of care. - Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs.
- Develop, implement, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of care.
- Gather data on physical, psychological, social, spiritual, and cultural factors and incorporate findings into the POC.
- Maintain up-to-date patient records with visits, contacts, problems, plans, actions, and goals that reflect changes as they occur.
- Deliver care using infection control measures (OSHA) and educate patients and families on care, medications, home safety, and related topics; promote self-care.
- Supervise and provide clinical direction to HHA/STNA/LVN staff to maintain quality and continuity of service.
- Assess and reassess pain; apply appropriate pain management strategies and educate on pain control.
- Arrange referrals to appropriate agencies and monitor cases for third-party payer requirements.
- Perform on-call responsibilities as assigned and communicate effectively with patients, families, team members, and physicians.
- Attend interdisciplinary team meetings and other required discussions to coordinate care.
- Perform other related duties as assigned or requested. Requirements
- Current state license as a Registered Nurse (North Carolina) – Required
- Education: Associate Degree in Nursing or Nursing Diploma – Required; Bachelor of Science in Nursing – Preferred
- Experience: 1–2 years RN experience in a clinical care setting – Required; 2+ years home health experience – Required
- Certifications: CPR certification – Required; CHPN certification – Preferred
- Special skills: Homecare Homebase Software Experience – Required; OASIS Experience – Required
- Licenses/credentials: Valid driver’s license – Required; Reliable transportation and insurance – Required
- Additional: Knowledge of medications and their administration; venipuncture and IV care; care for indwelling catheters and drains; safe use of durable medical equipment; ability to work independently and travel locally; ability to maintain patient confidentiality Benefits and Support
