Home Infusion Clinical Pharmacist

Home Infusion Clinical Pharmacist Pay Rate: $64.27 per hour

Home Infusion Clinical Pharmacist Compensation

• $64.27 per hour
• Benefits available to full-time employees after 90 days of employment

Home Infusion Clinical Pharmacist Schedule

• Monday-Friday
• Rotating shifts:
• • 8:00 AM – 5:00 PM
  • 8:30 AM – 5:30 PM
  • 9:00 AM – 6:00 PM
• Required evening and weekend on-call rotation
• Overtime available as needed

Home Infusion Clinical Pharmacist Responsibilities

• Review home infusion referrals for clinical appropriateness, including medication selection, dosing, route of administration, laboratory values, and patient-specific factors.
• Manage new patient admissions and refill authorizations for home infusion therapies.
• Perform medication reconciliation and provide patient counseling regarding prescribed therapies.
• Monitor laboratory results and adjust therapy recommendations as appropriate.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate patient care.
• Accurately enter prescriptions, complete documentation, and process claim adjudication using pharmacy software.
• Prepare and verify sterile compounded medications in accordance with USP standards and company policies.
• Ensure accurate labeling, lot tracking, and quality assurance for compounded medications.
• Supervise and support pharmacy technicians throughout daily operations.
• Assist with production workflow, medication dispensing, inventory management, and delivery coordination.
• Maintain compliance with state and federal pharmacy regulations while ensuring safe and timely medication delivery.

Home Infusion Clinical Pharmacist Requirements

• Active Pharmacist license in good standing.
• Home infusion pharmacy experience is required.
• Experience reviewing clinical referrals and managing home infusion therapies.
• Sterile compounding experience in accordance with USP guidelines.
• Experience with medication reconciliation, patient counseling, and laboratory monitoring.
• Ability to work independently in a fast-paced environment while managing multiple priorities.
• Strong communication and collaboration skills.
• Attendance is mandatory during the first 90 days.

Home Infusion Clinical Pharmacist Preferred Qualifications

• Experience using CPR+ pharmacy management software.
• Previous experience supervising pharmacy technicians.
• Knowledge of third-party billing and claim adjudication.
• Experience supporting operational workflow within a home infusion pharmacy.
