Responsibilities
- Assess home health and hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs, and document findings.
- Implement, develop, document, and evaluate the Plan of Care to ensure quality and continuity of care.
- Obtain data on physical, psychological, social, spiritual, and cultural factors and integrate into the Plan of Care.
- Maintain up-to-date patient records reflecting visits, problems, plans, actions, goals, and changes as they occur.
- Educate patients and families on care, medications, home safety, and self-care practices; apply appropriate teaching methods.
- Supervise and provide clinical directions to HHA/STNA/LVN to ensure quality and continuity of service; coordinate care among team members and agencies.
- Assess and manage pain, including education on pain management strategies; arrange referrals when needed.
- Perform on-call responsibilities and provide on-call services as assigned; communicate effectively with patients, families, and the care team.

Requirements
- Current North Carolina Registered Nurse license; required. CPR certification; required. CHPN certification preferred.
- 1–2 years RN clinical care experience; 2+ years home health experience required.
- OASIS experience required.
- Homecare Homebase software experience required.
- Education: ADN or Nursing Diploma required; BSN preferred.
- Driver’s license and reliable transportation required.
- Education and experience must support ability to work in a home health setting; flexible schedule and local travel required.
