- Setting: Skilled Nursing Facility (travel assignment) Responsibilities
- Conduct comprehensive PT evaluations and establish individualized treatment plans to address mobility, balance, gait, strength, and activities of daily living.
- Implement therapy interventions, monitor patient response, and adjust plans to maximize functional outcomes.
- Document assessments, treatment progress, and outcomes in the electronic medical record; communicate with the interdisciplinary team.
- Educate residents and families on exercises, safety strategies, and home programs to promote ongoing progress and independence.
- Collaborate with nursing, medical providers, and therapy colleagues on discharge planning and care coordination; ensure safe transfers and continuity of care. Requirements
- Doctor of Physical Therapy (DPT) degree or equivalent.
- Current physical therapy licensure or eligibility to obtain licensure for the assignment state.
- Ability to work effectively in a skilled nursing/long-term care setting.
- Strong verbal and written communication skills; proficiency with documentation and electronic medical records.
- Willingness to travel for a 13-week assignment and follow the posted shift schedule. Benefits and Support
